Applicants for admission must be 65 years
of age before application can be made. Applicants may be admitted to the Home, provided
that they are in such health of body and mind that the Home shall be able to provide adequate care.
The admission application is initially submitted to the Executive Director of the Home.
It is then referred on to the Admissions Evaluation Committee for consideration and
subsequent approval by the Board of Trustees.
Plans of Admission (brief synopsis of each)
- Boarding Plan - This is a monthly private-payment plan. No asset
transfer is required. The Home takes no responsibility for legal
representation as under the Trustee Plan. Applicant and/or applicant's
representative pledge that applicant's assets and income will be used
solely for payment of applicant's needs. Medicaid enrollment,
which occurs after assets have become depleted, is the responsibility of
the resident's agent. (Available to all applicants)
- Trustee Plan - Under this plan, the Home agrees to provide continuing care for the
duration of applicant's residence at the Home in exchange for all assets being conveyed to
the Home at admission. (Available only to Lutherans and Residents of the Home's
Independent Living Program. This plan requires assets sufficient
to pay for at least two years' care.) Those who do not meet this
requirement are eligible to apply under the Boarding or Assistance
Plans.
- Assistance Plan - Those applicants who are admitted to the Home under this plan must
have made application for and shall have been approved for Medicaid nursing home benefits
prior to admission.
All services and facilities of the Home are available to every resident equally without
regard to race, color, national origin or the plan under which admitted.
