Healthcare Facility Residents

NLHV is committed to providing excellent healthcare and quality of life for people 65 and older. The Health Care Facility addresses the needs of people seeking long term care; short term rehab to return home; or a trusted environment for caregivers needing respite assistance. Admission to NLHV is under one of two plans:  Boarding Plan   or  Assistance Plan . Selecting a skilled nursing care facility for long term care or a short term rehab stay is an important decision, but the process is not difficult. We’ve designed our application process to be simple and clear.

• Step 1    Tour the facility – 30 minutes to 1 hour depending on family need
• Step 2    Complete the Application Package – 30 minutes
• Step 3    Review Health and Medical profile by our Medical Staff – same day
• Step 4    Acceptance Call and schedule pre-admission meeting – 30 minutes
• Step 5    Admission Day – 1 hour
